For the purposes of establishing and maintaining credit, the undersigned submits the foregoing statement and information contained on this sheet, both written and printed, including supplemental sheets, if any, as being full, true and accurate statements of the applicant’s financial condition and related above matters, on the date stated. The applicant agrees to notify KC Financial, LLC, in writing immediately should there be any material change in its financial condition related to the above disclosures, and in the absence of such notice, or a new and full written statement, all information provided herein shall be considered as a continuing statement of the applicant’s financial condition and substantially correct. The undersigned hereby authorizes KC Financial, LLC, it’s assignees and/or designees to make inquiry into, to request, and to receive any information concerning my character, general reputation, current assets, credit (including, but not limited to, credit bureaus, credit reports from national credit bureaus and consumer reporting agencies), all financial and other information from creditor banks, and other information resources, which KC Financial, LLC, it’s assignees, and/or designees, deem relevant for the evaluating, granting and the collection upon the proposed borrowing. This authorization shall be effective from the date upon which this application is signed and shall extinguished automatically upon the full repayment of any granted borrowing request, if lending is so granted under this credit application. Pursuant to Title 12 US Code (CFR), I do understand that my information may further be transferred to financial institutions for evaluation of my credit worthiness and upon request, additional information as to the scope of this inquiry, if one is made, may be provided.
I understand that the completion of this document does not guarantee credit approval. It is further understood that the decision to extend credit will be at the sole discretion of KC Financial, LLC, and/or its associated credit partners.
